PATIENT INFO

We are dedicated to offering our new and returning patients the best care and to provide you with all of the important information you need to make healthy decisions.

What You Can Expect From Your First Visit?

Your first dental appointment generally includes an initial exam, x-rays and a cleaning. Your dentist will discuss your diagnosis and treatment options. If you are experiencing tooth pain, our dentists will evaluate the site area and determine the best course of action to alleviate pain.

Please assist us by providing the following information at the first visit:

  • If your previous dentist has taken x-rays, please request that they forward them to our office.

  • List of current medications

  • Photo ID

  • If you have dental insurance, please bring your insurance card so we are able to process your claims.  All insurance information is provided to our office at the time you schedule your appointment so staff can verify your benefits prior to your scheduled appointment.

Insurance Information

If you wish for us to bill your insurance company and have your benefits payable directly to our office, you must provide us your dental insurance information (card) to verify your eligibility and benefit provisions before your first scheduled appointment. Otherwise, you will be responsible to make full payment for services rendered and we will submit the insurance claim on your behalf to have the benefit payments sent directly to you.

We accept most major insurances, dental plans. Call our office at (860) 740-5096 to check and see if we are a provider for your plan!
